Homecare reablement
Homecare reablement is short-term support to help keep vulnerable people safe and independent in their homes.
We will work with you to develop confidence and skills to carry out daily living activities and continue to live safely and independently at home. This kind of support can continue for up to 6 weeks.
The help you will receive
We will:
- work with you to help you re-learn or regain the abilities you lost
- visit you to talk about your needs and what you want to achieve
- support you to help you achieve the things you care about
Who homecare reablement is for
You can get homecare reablement if you are:
- disabled
- frail
- ill
- have been in hospital
Care Quality Commission reports
